Subject: ATTENTION: Community Referrals to LA-Y.E.S.
Hello,
Louisiana Youth Enhanced Services (LA-YES) looks forward to building
partnerships and increasing capacity in the greater New Orleans area.
As part of the Cooperative Agreement, LA-YES is to provide mental health
services in five parishes: St. Tammany, St. Bernard, Jefferson,
Orleans, and Plaquemines. At this time, LA-Y.E.S. is accepting
referrals for service delivery. If you know of any youth ages 3-21
years of age in need of mental health services and treatment, please
contact Sanquinette Haynes, LA-YES Intake Coordinator, to become
enrolled in the program. Her contact information is shaynes at dhh.la.gov,
504-896-2625.
A review of the LA-YES Referral process is below. I have also attached
a referral form and information release form. If you have any
additional questions or need more information about LA-YES, please reply
to this email or call 504-896-2636. Thank you and we look forward to
expansion of service delivery into your parish!
Referral Process: How to refer children and youths to the LA-Y.E.S.
program
Please follow the steps below:
1. Contact our intake coordinator by calling 1-866-39LAYES.
2. The intake coordinator will send you a referral form that you will
need to fill out and fax in to our office at 504-896-2668.
3. The care manager supervisor will set up a meeting in order to do
a more in-depth screening with the parent(s) or guardian(s) of the
child as well as the child.
4. The care manager supervisor will then review the child's
information in order to determine if he/she is eligible. If eligible, a
care manager will be assigned to him/her.
